If you still experience problems completing the survey, please contact us by sending an email to info@ibdpartners.org. It is helpful if you can include a screen shot of the problem.


We do not support iPad, iPhone, tablets or smart phone platforms. The survey may work on these devices, however it is best if you can complete it from a laptop or other computer.

If you expect an email from IBD Partners Kids & Teens but you can not find it in your email box, please check your Junk email folder. If it is not there either, please wait for some time.

If you can not access the survey link, click HERE to access the Home page.

We use TLS to transmit data securely over the Internet, and we are certified by Starfield Secure Certification Authority. If your computer does not recognize the site's security, you will receive a warning similar to the warning below. Find your browser in the list below and follow the instructions. If the certificate has expired, please report this issue to info@ibdpartners.org.



Firefox: Tools => Page Info => Security => View Certificate
IE: Click the Lock icon , which is located to the right of the Address bar, then click "View Certificates".
Safari: Click the lock icon in the address field at the top of the Safari window.
Google Chrome: Click the lock icon in the address field at the top of the Google Chrome window; click "Connection" tab, click "Certificate Information".
Opera: Click the lock icon in the address field at the top of the Opera window; click "Details".
Netscape: Tools => Page Info => Security => View

Do not use the "Go Back One Page" button provided by the browser. Use the "Go Back One Page" link found to the right of the green "Submit" button at the bottom of the survey.
   

If the survey stops while you are completing it, wait for a few minutes and then log back in. If you still cannot access the survey, please report this issue to mailto:info@ibdpartners.org.

If you do not answer all of the questions on a page before submitting, a message will pop up. Click "Answer the missing questions on this page" to answer the questions highlighted in yellow before continuing.

     OR Click "Go to next page" to save the answers and go to the next page.



You can navigate the survey using the links on the left of the screen.



You can update your contact information by clicking on the links at the top of the dashboard.



Use the Scroll Bar on the right of the yellow question box to see all of the questions. If you are using and iPad, use your finger to scroll down inside the yellow question box.
